I falsified patient care records during night shifts at a residential unit. I was responsible for hourly welfare checks on residents who could not move independently. Instead of visiting each room, I signed the care log in advance and stayed in the office. The residents affected were elderly and sedated, and no one challenged the paperwork. I used the official clipboard and signed with the same pen used by day staff so it looked consistent. Management audited forms, not rooms. I was paid full shifts and received positive reports for reliability. Over months, multiple residents missed required checks. When a fall occurred, the records showed full compliance. I remained employed and the system accepted my version.
