Buried a Serious Complaint

A formal complaint alleging serious misconduct landed on my desk. I read it and understood the implications. Instead of escalating it, I filed it in a category that rarely receives follow-up. I never logged it for review. The complainant chased repeatedly and eventually gave up. No investigation ever happened. I avoided meetings and scrutiny by doing nothing. The system failed because I chose where to place the paper.
