Decisions were made without full information and maintenance was deferred. I benefited by avoiding budget overruns and scrutiny. When incidents occurred, they were framed as unforeseeable. The original reports remain archived but unread. The governance trail shows informed approval, despite the omissions, and no review has reinstated the warnings.
